本校流感疫苗接種時間訂於 114 年 10 月 14 日 (08:30–11:00)，當天需攜帶健保卡。
Our school’s influenza vaccination is scheduled for October 14, 2025 (08:30–11:00). Students must bring their National Health Insurance Card on the day of vaccination.

因應 E 化及減紙政策，疾病管制署推行「疫苗接種行政電子化系統 (NIAS)」，提供流感疫苗同意書線上簽署（網址或 QR code 擇一即可）。
In line with the digitalization and paper-reduction policy, the Taiwan Centers for Disease Control (CDC) has implemented the National Immunization Administration System (NIAS), which provides an online platform for submitting the influenza vaccine consent form. Parents may complete the process via the website link or QR code (either option is acceptable).

1️⃣ 填寫同意書期限

煩請家長於 114 年 9 月 30 日 (週二) 前完成「流感疫苗接種意願書」線上簽署。

系統截止後將自動關閉，未填寫者將視同不同意在校接種。

Parents are kindly requested to complete the online submission of the Influenza Vaccination Consent Form no later than Tuesday, September 30, 2025.

Please note: once the system closes after the deadline, it will no longer be accessible, and failure to complete the form will be regarded as declining vaccination at school.

Regardless of whether your child will receive the vaccination, you must complete the online reporting.

3️⃣ 修改意願期限

同意書填報截止日：114 年 9 月 30 日，截止日前可登入網頁修改接種意願。
The deadline for submission of the consent form is September 30, 2025. Before the deadline, you may log in via the above link to revise your decision regarding vaccination.

4️⃣ 接種當日攜帶物品

接種當天需攜帶 健保卡。
On the day of vaccination, students must bring their National Health Insurance Card.

5️⃣ 不同意在校接種之處理

若不同意在校接種，請於線上填寫原因（例如：將自行至醫療機構接種），學校將於當天發給補種通知單。
If you do not consent to vaccination at school, please indicate the reason online (e.g., “Will receive vaccination at a medical institution”). A make-up vaccination notice will be issued on the same day.
